The PHN Charitable Foundation (PHNCF) was founded in 2005 as the sole fundraising arm of the Primary Health Network (PHN) Pennsylvania's largest Federally Qualified Health Center (FQHC). PHNCF's mission is: "To build and support programs in the PHN service area that increase access to healthcare services and assist, through scholarships, with the pursuit of medical and allied health careers." PHNCF supports this mission by raising funds to build and sustain programs that are intended to improve the health and quality of life in the communities PHN serves. It's fundraising activities are driven by community support and engagement.

Programs Supported by Your Donations
Potential donations will benefit PHN Charitable Foundation's community-based efforts, which include our literacy initiatives in order to promote reading during formative years and throughout life . We have invested in "Reach Out And Read", a national non-profit that promotes integrating literacy into pediatric care. At each child's wellness visit from 6 months to five years, pediatric patients receive a book to take home (600 books have been distributed since the inception of this program). Another initiative we have invested in, is the "Little Free Library" program, offering an assortment of books to be accessed by the community for free, 24-7, similar to a formal library. Since it's inception we have distributed over 800 books to those throughout PHN's Western PA counties. The community has been very generous to our book drives with donations exceeding over 1000. Both of these programs are supported by fundraising/donations, and PHN Charitable Foundation aims to have the "Little Free Library's" in all of Primary Health Network's western PA communities.
Needs that Cannot beMet without Your Help
As communities recover from the COVID-19 pandemic, PHN Charitable Foundation maintains its focus on underserved areas and those disproportionately affected. Fundraising activities are curtailed during this recovery, but PHN Charitable Foundation strives to promote positive impacts that donations have on these populations. PHN Charitable Foundation aims to assist recovery by implementing supportive programs, which has been delayed, due to fundraising difficulties. These efforts prioritize PHN Charitable Foundation's literacy initiative, including: providing 24/7 cost-free book access via Free Little Libraries; diversifying selections through PHNCF’s Read In Color initiative; and adding new libraries in Mercer, Lawrence, and Trumbull counties along with the continuation of funding the "Reach Out and Read programs for our pediatraic patients. PHN Charitable Foundation will continue engaging in community collaborations to support this initiative.
